Injection Declaration Form

The Olympic Council of Asia enforces a needle-free policy at its Games. Whenever an athlete receives an injection during the period of a Games, the medical practitioner who administered it must submit this declaration. The OCA Medical Committee reviews every declaration.

How the declaration works

Who must submit

The medical practitioner who administered the injection to the athlete. No account is required.

Deadline

By no later than noon on the day after the injection was administered.

When no form is needed

If the injection was given by an Asian Games Organizing Committee practitioner and recorded in the medical encounter system, or for acupuncture and dry-needling.
